The American Association of Diabetes Educators did a survey among people with diabetes and diabetes educators to learn what people liked and didn’t like about the frst book. Conducted by the National Institute of Diabetes and Digestive and Kidney Diseases, National Institutes of Health, this important study provided scientifc proof that glucose control can help prevent or delay complications of diabetes. Ann Constance, Claudia Martinez, Hope Woodward, Margaret Fowke, Nancy Haynie- Mooney, Melinda Salmon, Mike Engelgau, and the Chattahoochee Nature Center also helped with the writing. Rick Hull, Diana Toomer, Melissa Stankus, and Kristina Ernst reviewed and edited the fnal version of this guide. You may even want to join a community group in which people share their stories and help others deal with their diabetes. The pancreas, an organ near the stomach, makes a hormone called insulin to help glucose get into our body cells. When you have diabetes, your body either doesn’t make enough insulin or can’t use its own insulin very well. Signs and Symptoms of Diabetes You may recall having some of these signs before you found out you had diabetes: ■ Being very thirsty. Scientists are learning more about what causes the body to attack its own beta cells of the pancreas (an autoimmune process) and stop making insulin in people with certain sets of genes. The pancreas of people with type 2 diabetes keeps making insulin for some time, but the body can’t use it very well. A national study show’s that people may be able to prevent orstudy show’s that people may be able to prevent or delay the onset of type 2 diabetes. Studies show that keeping your blood glucose (also called blood sugar) close to normal helps prevent or delay some diabetes problems. Through careful control, many problems such as eye disease, kidney disease, heart disease, nerve damage, and serious foot problems can be prevented or slowed. People who have type 1 diabetes as well as people who have type 2 diabetes can beneft by keeping their blood glucose levels closer to normal. To keep your glucose at a healthy level, you need to keep a balance between three important things: ■ What you eat and drink. When you eat dairy products (cheese, milk, yogurt, and others), choose those that have little or no fat or cream. Here are some things you can do to eat less sugar: – Eat more high-fber foods, like vegetables, dried beans, fruit, and whole grain breads and cereals. See pages 28–32 for more on ways to prevent problems when your blood glucose levels are too high or too low. If you want to include a drink in your food plan once in a while, ask your health care team how to do so safely. A Few Things About Diabetes Medicine If you take diabetes pills or insulin injections to control your diabetes, ask your health care provider to explain how these work. If you take other medicines that are sold with or without a prescription, ask your doctor how these can affect your diabetes control. If you inject insulin, your health care team should be able to tell you: ■ How to give yourself injections.

When farm workers receive what they consider to be a toxic level of exposure to pesticides with organophosphates, it has been found they have nearly six times the rate of depression as the general public (Stallones 2002). Particulate air pollution, a pervasive exposure in modern urban environments, has been found to alter brain structure and cause cognitive impairment and depressive symptoms. Mice exposed to pollutants at the same levels of modern city inhabitants were found to have not only depressed states, but elevated cytokine expression in the hippocampus and altered dendrite growth (Fonken 2011). The treatment for toxic exposure will vary depending on the substance but the first line of defense would be, if possible, removal of the offending material. In the case of occupational or 66 | Complementary and Alternative Medicine Treatments in Psychiatry habitat exposures, difficult choices may be involved requiring finding new employment or changing living quarters. Summary With allergies increasing and toxic exposures on the rise in our increasingly industrialized world, psychiatric symptoms from these environmental causes are also becoming more prominent. A wise physician, on the lookout for such risk factors, could save a patient years or even a lifetime of misdiagnosis and add years of more healthful living to what might otherwise be an existence of slow and mysterious decline. Breathing Technique, Mindfulness, and Yoga Christine Berger Abdominal Breathing Abdominal breathing, also called diaphragmatic breathing or belly breathing, is a core activity of meditation and yoga practices and an important therapeutic technique in its own right. Various Eastern religious and philosophical traditions cite the breath as a bridge connecting mind-body-emotions-spirit (Brown 2009). Anxious or depressed individuals breathe only from the upper chest, in a shallow fashion, whereas individuals who have an integrated mind-body system breathe deeply, from the diaphragm. Therefore, for clients with anxiety disorders or depression, it follows that breath training can serve as an empowering adjunctive treatment for these mental health challenges (Weil 2006). In fact, according to Philippott et al, cited by Brown and Gerbarg, changing breath patterns therapeutically “can account for at least 40% in feelings of anger, fear, joy and sadness” (Brown 2009). When practiced, it appropriately energizes the sympathetic nervous system on the inhale, and the exhale appropriately engages the parasympathetic system (Brown 2009). Proper breathing manages energy, breath volume, and adjusts other biological systems such as the endocrine, digestive, circulatory and neurochemical. Regulation of breath regulates heart function through the vagus nerve (Edwards 2008). The Impact of Breathing on Anxiety and Depression While Americans often think of yoga as a series of meditative postures, the system of yoga includes an emphasis on various breathing techniques which induce a variety of desired states. They were especially curious about the mechanisms of change from this form of breathing. The authors emphasize the importance that healthcare providers comprehend the research and clinical evidence of these therapeutic benefits. It consists of 4 parts: 3-stage slow resistance breathing (Ujjayi), bellows breath, om chanting, and cyclical breathing. Ujjayi has been shown to increase parasympathetic activity through vagal afferent inputs to the brain and improves heart rate variability. In addition, it improves low respiratory sinus arrhythmia, which has been associated with depression, anxiety, panic and obesity. Brown and Gerbarg also found evidence suggesting that yogic breathing has powerful physiological and psychological effects which may contribute to longevity (Brown 2009). A breathing training style called the Papworth Method, used to treat asthma and its accompanying anxiety, produced significantly less anxiety and depression in a treatment group as compared to a control group after five sessions and at a one-year follow-up (Holloway 2007). Meuret et al investigated whether breathing training for the treatment of panic disorder was effective or merely placebo effect. Although the nine studies they examined had methodological challenges, the authors concluded that breathing training had something to offer panic disorder treatment (Meuret 2003). Mindfulness Mindfulness, the simple act of being in the present moment, has been knows for centuries to be therapeutic. Mindfulness permits the individual to move out of the mind’s “noise,” thus reducing negative mental influences. Various techniques have been developed to assist individuals in focusing on the present, including outdoor activities, meditation, putting attention on the breath, and putting attention on a chant or sound. Myriad forms of meditation have been studied and practiced for thousands of years. Kabat-Zinn also emphasizes the factor of non- attachment to outcome as unique in clinical applications (Kabat- Zinn 2003). In addition, Baer conducted a theoretical summary and analysis of mindfulness as an intervention. She cites that Kabat-Zinn has hypothesized how meditation helps with anxiety, that “sustained, nonjudgmental observation of anxiety-related sensations, without attempts to escape or avoid them, may lead to reductions in the emotional reactivity typically elicited by anxiety symptoms” (p.

Conjunctival Cytology  Occasionally it is necessary to perform a conjunctival scraping for cytology to characterize the inflammatory response and evaluate for bacterial organisms or evaluate for neoplastic cells or parasitic organisms. This can be done with a chemistry spatula, a Bard Parker Scalpel handle tip (part that accepts the blade but with out a blade attached) a Kimura spatula or the butt end (end opposite the sharp blade end) of a scalpel blade. Conjunctival Biopsy  Biopsy of the conjunctiva for histopathology and or parasite evaluation can be performed on a standing horse after xylazine, auriculopalpebral nerve block and either a topical anesthetic or a subconjunctival (sublesional) injection of 2% lidocaine (0. At the time of the subconjunctival injection and when the tissue to be biopsied is picked up with a fine rat toothed forceps, the application of a lip or ear twitch is usually necessary and advisable. Avoid aggressive large biopsies of the conjunctiva, for prolapse of orbital fat could occur or suture the wound after the biopsy. Corneal Cytology  Sampling of a corneal ulcer bed for culture usually requires a scraping with a spatula to obtain an adequate amount of material. Ideally, this should be done prior to the application of fluorescein or topical anesthesia due to the possibility of a bacteriostatic/cidal effect from these chemicals. If a sample can not be obtained with out topical anesthesia then anesthesia can be used. Acknowledgments The development of this lecture note for training Health Extension workers is an arduous assignment for Ato Melkie Edris at University of Gondar Essentially, it required the consolidation and merging of existing in depth training materials, examination of Health Extension Package manuals and the Curriculum. They do not grow well, they become ill, many die or they do not grow up as clever, as healthy. Causes and consequences of poor nutrition are better understood now, and so are the ways to prevent and manage it. The underlying causes are insufficient household food security, inadequate childcare and insufficient basic health services in the community. It includes poor living conditions, lack of education, heavy physical work, and frequent childbearing. And the basic causes are economic structure, political and ideological superstructure. The mortality among preschool children is extremely high in developing countries in general and in Ethiopia in particular. It is quite clear that malnutrition in combination with infection, more often than not is, the cause of high morbidity and mortality in Ethiopia. Micronutrient deficiencies such as vitamin A, iodine and iron are the most prevalent in Ethiopia and it affects mothers and children at large. The entire efforts in the preparation of this lecture note require that the elements of nutrition should be understood by Health Extension 1 Nutrition Students and their Instructors for the implementation of nutrition interventions. Those, which regulate chemical process in the body, are the vitamins and minerals. It is the means of transportation for most nutrients and is needed for all cellular activities. Nutrition: - is the sum total of the process by which living things receive and utilize the necessary materials for survival, growth and maintenance of worn out tissues. Malnutrition: - is the condition that results from an imbalance between dietary intake and requirements. It includes under nutrition, which results from less food intake and hard physical work and over nutrition results from excess food intake and less physical activities. Diet: - is defined as food containing all the nutrients in a sufficient amount and in proper ratio. It prevents gastrointestinal disorders (gastritis, appendicitis, gallbladder stone and constipation) and metabolic disorders (diabetes mellitus, hypertension, ischemic heart disease and colon cancer). Dietary guidelines ƒ Eat a wide variety of foods ƒ Maintain healthy weight ƒ Choose a diet low in fat, saturated fat, and cholesterol ƒ Choose a diet with plenty of vegetable, fruits and grain products ƒ Use sugar in moderation ƒ Use salt and sodium in moderation ƒ If you drink alcoholic beverage, do so in moderation.
